Congratulations, either you are preparing to transfer positions or experiencing a change in your employment status.  In order to document this change please utilize the Employee Action Form ( EAF) located here :  Employee Action Form

 

An Employee Action Form should be submitted for any of the following activities:

Changes In:

  • Position
  • Work Location ( Property )
  • Hours Worked
  • Additional Compensation such as an auto allowance for associates assigned to multiple properties or phone allowance for on call associates.
